If you don't have Adobe Acrobat Reader, please download by clicking here. Print and fill out the Sebago Leadership Camp application and medical release form. Mail the application, along with full payment or a deposit of $100 (payable to SEBAGO LEADERSHIP CAMP) to:

Upon receipt of your application, you will receive an email notification of acceptance, in addition to other detailed information about the camp: departure and arrival information, packing lists (also available on this website), address and phone number of the camp, etc. You can also New applicants may be contacted by our staff for an interview prior to acceptance. We do not want cost to be an obstacle to any family. Please contact Bob Sylvain at 401-935-6576 if you need to make alternative financial arrangements for your son to attend camp.
